Documentation required for Active Duty, Retiree Renewal, and Expired ID Cards

Active Duty

  • Two unexpired forms of identification with at least one showing date of birth
  • AKO account needs to be established before issuance of CAC

Active Duty to Retiree

  • Retirement orders or DD 214
  • Two unexpired forms of identification with at least one showing date of birth

Retiree

  • Two unexpired forms of identification with at least one showing date of birth

Reserve Retiree (Gray Area) at the age of 60

  • Enrollment in DEERS
  • Notice of eligibility for retired pay at the age of 60
  • Two unexpired forms of identification with at least one showing date of birth

or

  • Enrollment in DEERS
  • Retired pay order showing they are retired with pay on the 60th birthday or later
  • Two unexpired forms of identification with at least one showing date of birth

Spouse

  • Sponsor needed

or

  • DD Form 1172 signed by a verifying official of an ID facility and sponsor

or

  • DD Form 1172 signed by Sponsor and Notary
  • Two unexpired forms of identification with at least one showing date of birth

Unmarried Widows

  • ID card
  • Death certificate
  • Two unexpired forms of identification with at least one showing date of birth

Child/Ward

  • Sponsor needed

or

  • DD Form 1172 signed by a verifying official of an ID facility and sponsor

or

  • DD Form 1172 signed by sponsor and notary

Note: ID Cards are needed for children at the age of 10, unless sponsor is divorced or dual military, children will receive ID cards before the age of 10.

Children/Ward - Full time Students (Age 21-23)

  • Sponsor needed
  • Sponsor providing over 50% support of child
  • Letter from the school registrars office, certifying child is enrolled in a full-time course of study, and expected date of graduation
  • Two unexpired forms of identification with at least one showing date of birth

Un-Remarried Former Spouse

  • Two unexpired forms of identification with at least one showing date of birth
  • Spouse has not remarried
